Job Title: Case Manager
Department: Support Services
Essential Job Functions
  • Provides specific case management services to individuals in day programs.
  • Coordinates with community resources needed (medical, psychological, vocational, recreational and supported services) based on the assessed needs of the people we support.
  • Assists individuals in becoming eligible for financial and other resources, which will enhance their independence.
  • Maintains individuals’ files and records. Request needed records from families, agencies, and other professionals.
    • Coordinates and facilitates Person Centered Panning meetings with the Coordinators of Community Services
  • Prepares thorough social service summaries, a review of all needed medical assessments and other necessary paperwork in conjunction with the Person-Centered Plan and in accordance with DDA regulations. Compile all pieces of IP and review for consistency.
  • Reviews individuals’ services on a quarterly basis and request needed services.
  • Assists families, staff and other relative persons in understanding the changing needs of the individual. Acts as an advocate for the people we support and safeguards their legal rights.
  • Intervenes in daily crisis situations to assist individual in working through time limited problems, as well as ensuring the most appropriate services are received.
  • Assists with the coordination of transportation for the individual.
  • Provides occasional transportation to meetings and medical facilities.
  • Attends regular meetings, including Core Review, Department, Admissions Review and Discharge, Case Management and committee meetings.
  • Completes annual reviews for all day program Case Managers.
  • Staff will be required to utilize agency electronic systems for communication, progress notes, incident report filing, etc.
  • Attend and actively participate in all scheduled meetings.
  • Complete all state and agency required documentation in accordance with stated guidelines and deadlines.
  • Attend and actively participate in all scheduled meetings.
  • Check, and respond to emails daily to maintain proper communication
  • Other duties may be assigned by the Director of Support Services.

Education:
Bachelor’s degree in psychology, sociology, social work, human services, or other related field and 2 year’s experience with individuals with disabilities preferred. Associates Degree will be considered with substantial professional experience.
Training and Experience:
Preferred DDA core competencies, Mandt Systems Training, CPR / First Aid, MTTP
Experience working with people with IDD/DD preferred but not required.
Physical Requirements: Person must be able to walk, stand, and sit for up to 8 hours. Person must have the ability to lift up to 50 lbs. or more based on assigned supported person (using health and safety lifting / turning techniques or assistive devices).
Person must be able to successfully demonstrate all Mandt Systems training physical interventions and techniques, to include assisting supported person to standing, providing assistance with ambulation, physical releases, and physical restraints.
