Position Overview
Our client is seeking a dedicated and skilled Physical Therapist to join their Assisted Living Facility outpatient rehabilitation team on a full-time basis. The successful candidate will be responsible for providing exceptional care by assessing, diagnosing, and treating individuals with various musculoskeletal injuries and conditions. This role requires a commitment to patient-centered care, a passion for helping others improve their quality of life, and the ability to work collaboratively within a multidisciplinary team.
You have the opportunity to work 1:1 with a mostly geriatric population and provide care for a range of orthopedic conditions, neurologic movement disorders, post-op cases, and balance program

Key Responsibilities
- Evaluate patients' physical conditions and develop individualized treatment plans based on clinical findings.
- Implement therapeutic interventions to address patient needs, including manual therapy, exercise prescription, and modalities.
- Monitor patient progress and modify treatment plans as necessary to achieve optimal outcomes.
- Educate patients and their families on exercises, injury prevention, and lifestyle modifications to promote overall health and well-being.
- Maintain accurate and up-to-date patient documentation in compliance with legal and professional standards.
- Collaborate with other healthcare professionals to ensure comprehensive care.
- Participate in professional development opportunities to stay current with evidence-based practices.
- Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
- Current state licensure as a Physical Therapist (or eligible for licensure).
- Experience in outpatient settings preferred, but new graduates are welcome to apply.
- Excellent communication and interpersonal skills, with the ability to motivate and educate patients effectively.
- Proficiency in electronic health records and clinical documentation software.
