As a Geriatric Case Manager you will play a crucial role in coordinating and managing the care of our client’s elderly patients. Leveraging your expertise in geriatric nursing and compassionate approach, you will collaborate with interdisciplinary teams to develop and implement comprehensive care plans tailored to meet the unique needs of each individual.
Responsibilities:
- Conduct comprehensive assessments of elderly patients, including physical, cognitive, and psychosocial aspects, to identify their care needs and preferences.
- Develop personalized care plans in collaboration with patients, families, caregivers, and healthcare providers, ensuring alignment with best practices and regulatory standards.
- Coordinate and facilitate communication among healthcare team members, including physicians, nurses, therapists, and social workers, to optimize patient care delivery and outcomes.
- Monitor and evaluate patient progress regularly, adjusting care plans as needed to promote optimal health and well-being.
- Advocate for the rights and preferences of elderly patients, ensuring their voices are heard and respected throughout the care continuum.
- Provide education and support to patients, families, and caregivers on geriatric health issues, disease management, medication adherence, and community resources.
- Collaborate with community agencies, long-term care facilities, and other healthcare providers to ensure seamless transitions of care for elderly patients.
- Maintain accurate and up-to-date documentation of patient assessments, care plans, interventions, and outcomes in electronic health records (EHR) systems.
- Stay abreast of current trends, research, and developments in geriatric care and case management, participating in continuing education and professional development activities as required.
Requirements:
- Active Registered Nurse (RN) license in New York.
- Bachelor of Science in Nursing (BSN) preferred.
- Minimum of 5 years of clinical experience in nursing required.
- Previous experience with the geriatric population highly preferred.
- Strong assessment, critical thinking, and problem-solving skills
- Excellent communication and interpersonal skills, with the ability to interact effectively with diverse populations
- Ability to work independently and collaboratively within a multidisciplinary team
Please note- The annual base salary range is $75k to $100k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
