PURPOSE OF POSITION:
Responsible for conducting and coordinating the complete resident comprehensive assessment, which includes the Minimum Data Set (MDS 2.0) and Maryland Monthly Assessment. The coordination of appropriate participating professionals for the purpose of conducting initial and comprehensive, accurate assessments of each resident that allows the resident to reach his/her highest practical physical and mental and psychosocial functioning.
QUALIFICATION:
- Current MD RN/LPN license, BLS certification.
- Strong oral and written communication skills.
- Graduate of accredited Nursing School.
- Long Term Care experience of at least one (1) year and acute care at least two (2) years knowledge of current Federal and State reassessing residents and developing care plan.
POSITION RESPONSIBILITIES
Responsibilities related to Resident Assessment
- Coordination of all required Resident Assessment Instruments (RAI) per state and federal regulations to ensure timely completion.
- Timely, accurate completion of all aspects of nursing portions of required RAI’s.
- Timely transmission of all required RAI’s per state and federal regulations.
- Maintenance of an RAI calendar, accessible to members of the Interdisciplinary Team (IDT), which provides clear, concise, up-to-date information on completion deadlines for each phase of the RAI process for each applicable resident.
- Monitoring of ancillary department sections of the RAI for timely completion, including notification of the individual departments if deadlines are in danger of not being met by the departments.
- Prompt notification of IDT members when a change of condition RAI is initiated.
- Monitoring of input into each Minimum Data Set (MDS) to ensure accuracy and consistency of responses across the interdisciplinary assessments, acting as a facilitator to help resolve disagreements among team members.
- Mentoring and teaching of nursing and ancillary staff to ensure that all team members are aware of and competent in their roles in the RAI and the Prospective Payment System (PPS).
- Maintaining smooth, seamless, timely communication with Sharp Health Care billing department regarding residents’ Medicare status and RUG categories.
- Maintaining a current knowledge base regarding state and federal regulations relative to the RAI process and PPS.
- Notification to the Unit Manager of any barriers to timely, accurate completion and transmission of RAIs in order to prevent missing any deadlines.
Responsibilities Related to Care Planning
- Coordinating and facilitating interdisciplinary Resident Care Plan Conferences (RCPC) on or about the 10th day of admission for each resident, ensuring that the resident and responsible party receives written invitation and notification of the meeting, specifying time and place.
- Coordinating and facilitating interdisciplinary RCPCs as situations occur on a day-to-day basis.
- Active participation in the development of a comprehensive, individualized, interdisciplinary plan of care for each resident.
- Ensuring that care plans are completed, updated, and accelerated to reflect an interdisciplinary approach to the resident’s changing needs.
Responsibilities Related to Medicare Utilization Review
- Review of each Medicare resident’s case of the day of admission (except weekends) to ensure appropriateness of Medicare coverage.
- Active interaction with IDT in UR rounds on Mondays, Wednesdays, and Fridays to evaluate residents’ status and needs related to Medicare skilled needs and potential for RUG category changes.
- Conducting daily, concurrent utilization review via chart audit in order to ensure that:
- Residents receive the care and services they need in a timely manner;
- Change of condition RAIs are initiated when appropriate;
- Chart documentation, including nursing and ancillary progress notes, care plans, and C.N.A. notes, reflects the need for care and services the resident received;
- Discharge planning proceeds smoothly related to resident’s progress and Medicare coverage status;
- Medicare coverage is discontinued when resident no longer meets criteria.
- Ensuring that Medicare certification/recertification forms are signed by physicians per regulations.
SPECIAL REQUIREMENTS
- Licensed as a registered nurse or licensed practical nurse in the State of Maryland.
- Basic Cardiac Life Support certified at the time of hire and annually thereafter.
- Must be able to communicate effectively in English, both verbally and in writing.
- Must be able to physically turn, lift, and support the weight of the adult residents safely during the provision of routine care and during emergency situations.
