JOB SUMMARY
The Assistant Director of Nursing is responsible for assisting the Director of Nursing with the planning, implementation, delivery, and supervision of direct clinical services provided to clients and families, the interdisciplinary team coordination of the Hospice program, and the effective and efficient use of resources. The position may also serve as part-time RN Case Manager. This position reports to the Director of Nursing.
JOB SPECIFICATIONS
· Registered Nurse (RN) with a current license to practice in the state of operation.
· Minimum of two (2) years nursing experience, preferably in a Home Health/Hospice setting within the past five (5) years.
· Demonstrates ability to manage all aspects of terminal care with expertise in pain and symptom management and hospice nursing.
· Demonstrates skill in administrative performance, including expertise in both written and oral communication, supervision of staff, documentation requirements, service coordination, case management and supportive, sensitive approach in response to needs of staff and client/family.
· Maintains a knowledge base of Medicare, Medicaid, state, and Joint Commission requirements.
- Schedule flexibility and travel required.
- Must have valid driver’s license, vehicle insurance, and reliable transportation.
ESSENTIAL FUNCTIONS
· Participate in the admission process by performing hospice consultations and/or review of admission documentation to assess for appropriateness of admission to hospice care.
· Communicates with physicians to obtain orders for admission and recertification.
· Maintain current recertification schedule to be distributed to all staff in timely manner to maintain CMS compliance.
· Coordinate/schedule assignments of staff members to clients according to needs and level of care.
· Assist in supervising the planning, coordination, and delivery of the interdisciplinary team interventions.
· Assist in supervising the interdisciplinary team in the review and revision of the client/family plan of care every 14 days at IDT meetings.
· Participate in documentation review to assure that the hospice clients continue to meet qualifying criteria for care.
· Assist in implementing and monitoring compliance with agency policies and procedures related to clinical services as directed.
· Serve as a member of the clinical management team in order to address problems associated with the delivery of clinical services and staff development needs.
· Assist in assuring documentation and timesheet compliance as delegated by D.O.N by performing record review.
· Participate in QAPI program as directed by D.O.N and policy and procedure review.
· Participate in clinical supervision of nursing personnel, certified nursing assistants, and homemakers, such as conducting appropriate supervisory visits and competency evaluations as appropriate.
· Participate in orientation of new personnel or direct-care volunteers, as delegated.
· Assist in financial review of clinical operations.
· Assist with coordination of services in association with contracted providers, and analysis of their performance.
· Participate in In-Service education for ACH staff, community, and facilities.
· Update and maintain current client reports, census tracking, and comprehensive client reports.
· Maintain current knowledge of accreditation agencies requirements.
· Carry part-time case load as assigned.
ADDITIONAL EXAMPLES OF WORK PERFORMED
- Accompany Nursing staff on patient consultations as needed.
- Respond to staff, patient, and family needs, which may be after hours or on weekends.
- Participate in on-call rotation for after hours and weekends.
· Performs related duties as assigned or required.
WORK ENVIRONMENT AND PHYSICAL DEMANDS
- Duties are primarily performed in patient residence or an office environment and require sufficient personal mobility and physical capability to permit employee to function in these environments.
- Must be able to lift up to twenty (20) pounds.
- Required to work irregular hours including evening and weekend availability.
- Travel to client residences and other locations required.
