As a Nurse at Avalanche Care, you will play a pivotal role in delivering high-quality healthcare services to our patients. Your primary responsibility will be to establish and maintain comprehensive care plans for your assigned caseload, ensuring that each patient receives personalized attention and support tailored to their unique needs.
Key Responsibilities:
- Conduct thorough assessments, reassessments, and aide supervisions in patients' homes, adhering to appropriate bag techniques and demonstrating expertise in infection control practices.
- Orient and supervise Home Health Aides (HHAs) on the plan of care, both during in-home visits and through remote means when required.
- Provide assistance with orientation and in-servicing of aides in the office, ensuring compliance with company policies and procedures.
- Collaborate with the interdisciplinary team to develop and implement comprehensive care plans that promote optimal patient outcomes.
- Maintain accurate and up-to-date documentation of patient assessments, interventions, and progress notes.
- Communicate effectively with Case Managers and Nurse Case Manager Assistants, providing timely updates on visit schedules, missed visits, schedule changes, and potential discharges.
- Participate in ongoing education and training to enhance clinical skills and stay abreast of industry best practices.
Qualifications:
- Valid RN License in New York
- Minimum of 1 year of experience in home healthcare or a related field
- Strong clinical assessment skills and knowledge of infection control practices
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in electronic medical record (EMR) documentation preferred
- Reliable transportation and valid driver's license
Salary and Benefits:
- $100 per patient assessment
- Admin hours are 4 hours per week. $50 per admin hours.
