Essential Duties and Responsibilities:
- Provide initial/reassessment nursing assessments on all clients, following the guidelines set forth by the Department of Health
- Participate in audits by Agency and/or regulatory bodies.
- Ability to create a Plan of Care (POC) that will fit the client’s needs.
- Conduct a minimum of 23 assessments per week.
- Ensure timely Start of Care visits in Manhattan and the Bronx.
- Travel to the Bronx and Manhattan boroughs, with occasional work in Brooklyn and Queens.
- Create and adhere to a personal schedule to guarantee timely visit completion.
- Confirm visit completion daily and submit visit paperwork within 72 hours.
- Other responsibilities as assigned.
- Must hold a valid NYS RN License
- Home Care Experience preferred.
- Excellent communication skills
- Compassionate, Caring & Understanding
- Flexibility to accommodate last-minute visit requests.
- Bilingual preferred; Spanish-speaking is required.

Pay Range: The hiring range for this position is $110,000.00-115,000.00 annually. Various factors will determine final compensation such as a candidate’s years of relevant work experience, skills, certifications, and location.
