Overview: The RN MDS Nurse is responsible for gathering information on a healthcare facility's current and future patients for future assessment, including physical and mental states. The RN MDS (Minimum Data Set) Department Director has overall responsibility for ensuring accurate, and timely completion of the RAI (Resident Assessment Instrument) process. The RN MDS Department Director must understand both how to correctly code the MDS and how the MDS data is being used in multiple government programs. The RN MDS Department Director has overall responsibility for case management of all skilled residents, both Medicare Part A and managed care. The RN MDS Department Director must be able to provide organizational data to support quality, regulatory, and financial outcomes. The RN MDS Department Director must ensure that the company complies with accuracy and timeliness standards as listed in Federal and State Regulations and the RAI Manual. Each MDS Coordinator is responsible for an assigned set of resident rooms. The RN MDS Department Director has department supervisory responsibilities as assigned by the facility Administrator.
Job Responsibilities:
· The RN MDS (Minimum Data Set) Coordinator has overall responsibility for ensuring accurate, and timely completion of the RAI (Resident Assessment Instrument) process.
Specific duties for each set of assigned rooms include:
· Review all admissions for completeness of the electronic record. This includes opening assessments in accordance with Federal requirements as listed in the RAI Manual.
· Prepare assessment schedule and notify all disciplines of said schedule.
· Ensure that resident diagnosis coding is complete; review during completion of both scheduled and unscheduled MDS assessments. Ensure the diagnosis list is correct for billing and includes therapy diagnoses. Review with physician and clarify any diagnoses requiring documentation.
· Complete and document Resident interviews as required by Federal and State agencies.
· Complete MDS assessments in accordance with the RAI Manual. Be able to stand in for other departments if needed to include all interviews and MDS data entry.
· Lock MDS Assessments and sign for completeness- this is an RN-only function
· Ensure all skilled hard copy and electronic tracking tools and spreadsheets are updated per company policy
· Ensure resident records are complete; liaise with nursing and other departments as needed to coordinate completion of records and data tools.
Prepare and track electronic paperwork submissions to federal and state agencies. This includes the MDS and Michigan LOCD. Maintain a hard or electronic copy of all Final Validation Reports for 7 years.
Liaise with discharge planners, social services, therapy, admissions, clinical, and leadership staff to ensure good discharge planning and promote positive outcomes.
· Assist with facility audits and inspections including claims reviews, ADR (Additional Documentation Requests), and state/federal surveys. This includes updating the CMS 671/802.
· provide the CMS reports from CASPER (Certification and Survey Provider Enhanced Reports) and iQIES(Internet Quality Improvement and Evaluation System) to the Administrator, Director of Nursing, and leadership. Each of these reports provides valuable feedback and can also inform the quality improvement efforts of the facility. Reports include but are not limited to:
Qualifications:
· MDS RN must understand both how to correctly code the MDS and how the MDS data is being used in multiple government programs.
· Ensure that our company complies with accuracy and timeliness standards as listed in Federal and State Regulations and the RAI Manual
Education/Certification
· Licensed Practical Nurse from Accredited School in compliance with regulations of the state.
· Current CPR, First Aid, and AED certifications
