Required Shift: 4-10 hour shifts w/alt Sundays

Job Summary:

Coordinates and completes the referral and admission process to ensure patients smoothly transition to hospice services.

Essential Job Functions:

  • Routinely meets individual goals, and assists the team in meeting departmental goals set by the Director of Business Development.
  • Notifies referral sources of changes in processes or procedures.
  • Meets with physicians and healthcare facilities routinely to maintain open communications.
  • Develops and maintains good working relationships with physicians, hospital and nursing home staff and other referring agencies.
  • Provides accurate information on Avow services and hospice benefits.
  • Obtains appropriate documentation from referring physicians.
  • Assesses the physical and emotional needs of the patient/family and provides the framework from which the comprehensive care plan is developed.
  • Completes comprehensive assessment of patient/family needs at the time of admission.
  • Provides emotional support for the patient/family during the referral process.
  • Identifies patient/family educational needs and develops a teaching plan as needed.
  • Provides patient care as needed including catheter insertion, wound care, personal care.
  • Coordinates the hospital discharges as needed to support the clinical liaisons.
  • Assesses the need for medical supplies, medications and durable medical equipment.
  • Obtains health insurance and Medicare/Medicaid information. Submits to Billing Specialist for verification. Reviews billing procedure with patient/family prior to admit and answers related questions.
  • Initiates process and documentation of the physician certification of terminal illness.
  • Completes documentation in the EMR at point of care.
  • Communicates with the Patient Access Specialists to keep them updated on who is admitted and their availability.
  • Collaborates/communicates any special needs/situations within the family, home environment.
  • Maintains a documented tickler file to follow up with all prospective clients that are not admitted.
  • Is available for holiday, on-call, weekend, illness and vacation coverage as needed.
  • Performs other duties as assigned by the Patient Access Manager or designee.

Education/Experience: Associate degree required; Bachelor's degree (B.A.) from four-year college or university preferred; Graduate from accredited school of nursing.

Certificates, Licenses, Registrations: Valid driver’s license Florida Registered Nurse License CPR certification CHPN preferred, required within two years of hire or 1 year after transfer into position.

Computer Skills: Strong computer skills including database software and general knowledge of e-mail/Outlook. Ability to work and document within an electronic medical record.

Supervisory Responsibilities: This job has no supervisory responsibilities.

Language Skills: Effective communication skills of reading, writing, and speaking in English.

Math Skills: Math skills including addition, subtraction, multiplication, division, and percentages/fractions.

While performing the duties of this job, the employee may be exposed to outside weather conditions. The noise level in the work environment is usually moderate.
