What are the responsibilities and job description for the Transition Coordinator position at Axion?
Transition Coordinator - Large non-profit organization seeks a Transition Coordinator to assist in development plan preparation for each client. This position is located in West Orange, NJ.
You will earn $45,000 per year and work Monday through Friday from 9AM to 5PM, with some overtime on weekends, when needed. Plus, you will enjoy weekly pay and health benefits.
As a Transition Coordinator, you will be responsible for assessing physical, communication, transportation and financial barriers for clients and requesting accommodation, when necessary. You will also be expected to:
- Assist with resumes and job applications as well as prepare clients for job interviews and orientations.
- Observe clients on the job site and provide training assistance, as needed.
- Provide feedback and assistance to clients regarding job performance.
- Provide instruction in self-advocacy and crisis intervention, if needed.
- Document client and employer contact, activities and outcomes in Foothold.
- Attend Foothold training and other professional development training.
- Advocate on behalf of client in both crisis and job advancement, including salary, work conditions, and benefits.
- Input current employee changes for all clients.
- Report all contact of prospective employment partnerships and clients daily.
To qualify for the Transition Coordinator opportunity, you must have at least two (2) years of experience working with adults with learning and developmental disabilities. In addition, we require you to have:
- Have a high school diploma or GED.
- A Valid New Jersey driver's license.
- A valid CPR/First Aid Certification.
- Prior CDS Trainings.
