Network Partner Job Description –
The Nurse-Family Partnership (NFP) Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. Nurse Home Visitors (NHV) are pivotal in transforming the lives of vulnerable babies, mothers and families by implementing and maintaining the fidelity of the NFP model. Our goal is that all children are healthy, families thrive, communities prosper, and the cycle of poverty is broken. Nurse home visitors report to the NFP Nurse Supervisor.
• Provides home visits to women and their families eligible for the NFP Program.
• Adheres to nursing process and the NFP model of home visitation.
• Carries a caseload of up to 25 clients per 1 FTE (full time position).
• Self-reflection- Successful NHVs actively work to understand and address their personal reactions to client, welcome feedback non-defensively, and make good use of the reflective supervision provided.
• Teaching- Fundamentally enjoy teaching or contributing to the betterment of another person’s wellbeing.
• Optimism and Hopefulness- NHVs believe people can change and improve their lives. This is known as a “growth mindset” about people vs. a “fixed mindset” about people. NHVs with a growth mindset believe that people can learn, change, grow and improve.

• Contribution to Public Health- Successful NHVs often describe their commitment to serving the greater good of their communities and appreciate the public health implications of this work.
• Must be able to work a flexible schedule, possibly including some evening and weekends depending on needs of families served.
• Able to complete duties under stress, deadlines, and while attending to multiple tasks simultaneously.
• Occasional prolonged computed related exposure, as well as sitting and standing at workstations for long periods of time.
• Work assignments will involve travel throughout designated areas to multiple locations, including families served, government and private agencies and partners.
Job Qualifications
• BSN, or willingness to work toward BSN preferred.
• Current RN license (in good standing) required.
• Fully vaccinated for COVID-19 required.
• Current CPR required.
• Two years recent experience in maternal/child health, public health, home visiting or mental/behavioral nursing preferred.
• Home visiting experience preferred.
• Excellent written and verbal communication skills.
• Basic computer skills.
• Valid driver’s license and insured automobile required.
• Able to travel to Denver for face-to-face education sessions, and able to participate virtually for those offered virtually.
Looking for a Full Time nurse and a Half Time nurse. Serve six counties in Northeast Colorado: Yuma, Washington, Phillips, Sedgwick, Logan, and Morgan
Salary $32-38/hr based on education and experience.
