Overall Summary

 

The Social Service (SS) Manager is responsible for making significant decisions on how Services is performed across service programs within BACH: its purpose, function and role, and for making commitments and decisions that require the expenditure of significant facility specific budget.   The SS Manager will manage all nonclinical support services through a team of supervisors that support the delivery of basic health services and facilitate access to comprehensive patient care as well as social services.  The purpose of SS is to increase access to care and ensure patients full health center participation for all medically underserved patients.   SS services are a critical extension of the Patient Centered Medical Home care team.

 

Essential Responsibilities

 

The SS Manager plans, develops, organizes and monitors the supervision of day-to-day operational activities including case management, benefit counseling and eligibility assistance, health education and supportive counseling, interpretation, outreach, transportation, and education of patients and the community regarding the availability and appropriate use of health services at BACH. Essential responsibilities also include:   

 

  • Hires, trains, supervise, and evaluates ESS staff.
  • Works closely with and serves as liaison to social service agencies, and referral sources.
  • Work with appropriate department managers to: a) establish a system of tracking and monitoring services in their departments, and b) identify and address staff training needs that will assure that the  & Social Services are monitored.
  • Works with department managers to ensure that Social Services are documented and maintained for grant compliance, UDS reporting and utilization outcomes and effectiveness.
  • Develops educational materials and conducts presentations on BACH programs in Spanish and English. Conducts presentations in English plus additional languages.
  • Evaluates and improves BACH health education programs.
  • Collaborates with staff in all aspects that contribute to marketing, outreach and education of BACH’s services.
  • Develops, reviews, implements, maintains and monitors the outreach department’s policies and procedures.
  • Monitors staff for safety and compliance related issues, as well as effective customer service technique.
  • Tracks and evaluates program activities for grant compliance, reporting requirements and utilization review.
  • Develops and has oversight of the Social Services department’s budget and business plan.
  • Participates in various community and advocacy activities for the agency.
  • Works a flexible schedule to include evenings and weekends as necessary.
  • Manages and implements various program-related grants under general supervision of the Director of Administrative Services (DAS)
  • Works with planning and development department to develop grant proposals and plan activities related to Social Services, as needed.

 

A key element of  Social services is their relation to patient knowledge, skill, resource, and environmental deficits which prevent patients from accessing the prevention and health care services they may need.  Subsequent reporting shall address how staff and programmatic actions reduce each barrier and improves access to the needed services. Operationally, increased knowledge; reduced wait time, increased utilization of prevention services, increased conversion from uninsured to fully insured and consistent access to information of upcoming health opportunities are examples of desired outcomes of a highly functioning ESS Department.

 

The ES Manager is responsible for ensuring a standard process of services data collection, across all integrated functional areas, with the objective of enhancing information in support of the Triple Aim model, quality measures, as well as the cost and level of resources required to provide Social services, and justify the need for adequate funding for these critical services.  Overall, the documentation of data and examination of outcomes from Social services will support improvement of access, health outcomes and identify and reduce health disparities for underserved populations.

 

Secondary Responsibilities

 

  • Attend workshops, training, and meetings as needed, and as requested.
  • Perform other duties as assigned by supervisor.

Qualifications


Qualifications:

 

Required education, experience, training

  • Bachelor’s Degree in related field.
  • Minimum of two years prior work related experience in health education environment, including development of educational training materials, programs etc.
  • One year supervisory experience or three years in a lead role.
  • Oral and written bi-lingual communication (English and additional language required) 
  • Proficient in Microsoft Office products including graphics, Adobe Photoshop, Illustrator, Power Point, Excel, Word or equivalent software products.

 

Preferred education, experience, training

  • Community health center experience.
  • Master’s Degree in related filed such as MPH, MPA, etc.

 

Certifications/Licenses:

  • Valid California State Driver’s License and proof of current automobile insurance.
  • Health Exchange and other health plan eligibility certification. If not currently certified, BACH will assist in training within 90 days of employment.

  • Participates in development, implementation and maintenance of policies, objectives, short-and long-term planning; develops and implements projects and programs to assist in accomplishment of organizational objectives.

Work Conditions:

 

Travel locally to visit other sites, may require a day or overnight trip to attend events; workload may be demanding at times; require professionalism and confidentiality.

 

Physical Demands:

 

This position requires:

  • Sitting, walking, and standing associated with a normal medical/clinical office environment.
  • Regular bending and stretching; and lifting. 
  • Manual dexterity used to include, but not limited to, standard office equipment, and computer keyboard.
  • Traveling to work sites including some exposure to outside elements at outdoor fairs.
  • Presenting information in front of a group.
  • Reading and writing in order to review records for accuracy.
  • Moderate physical activity. Requires handling of average-weight objects up to 20 pounds or standing and/or walking for more than 4 hours per day.

 

Supervisor:

Managerial personnel (supervisors, managers, directors, executive management) who are exempt or non-exempt and who are responsible for use of independent judgment to make personnel decisions or to recommend personnel decisions. Personnel decisions include, but are not limited to: hiring, promoting, transferring, evaluating, rewarding and terminating employees.
