Summary

Assesses potential residents for appropriateness of nursing home placement based on level of care needed. Coordinates with Admissions Coordinator on new admissions and Coordinates Care Plan meetings with staff, residents and their families.
MDS Nurse Job Duties

Coordinates with Admissions Coordinator on new admissions and Coordinates Care Plan meetings with staff, residents and their families.
MDS Nurse Job Duties
- Tracking discharge & re-entry of residents.
- Completion of PAE in a timely manner.
- Maintenance of MDS schedule within appropriate time frame.
- Verifies MDS is completed within mandated time frame. Signature on form attests form is completed.
- Update matrix & roster once a week.
- Verify supporting documentation for MDS.
- Ensure appropriate skilling factor on nursing documentation.
- Responsible for understanding the PPS & PDPM payment models.
- Assesses potential residents for placement based on level of care.
- Completes nursing assessment section of MDS timely.
- Inputs data from MDS hard copy into the computer.
- Reviews triggered RAPS related to nursing & assists other disciplines to complete RAPS.
- Completes any additional resident assessment information necessary to provide clearer picture of resident condition.
- Educates the nursing staff of use of MDS & PDPM documentation.
- Sends Care Plan letters to families in a timely manner.
- Coordinates Care Plan conference.
- Addresses problems related to nursing care of residents.
- Writes nursing specific portions of Care Plan.
- Must be licensed nurse in good standing & currently licensed by the state.
- Must have the ability to read & write & follow oral & written directions at a level necessary to accomplish the job.
- Must be able to relate positively & favorable to residents & families & to work cooperatively with other associates.
- Must be capable of maintaining regular attendance.
- Must meet all local health regulations.
- Must be capable of performing the essential job functions of this job, with or without reasonable accommodations.
