Summary:
Assignment of ICD-10 and CPT codes to diagnoses and procedures in inpatient and outpatient medical records. Apply knowledge of coding principles, regulations and guidelines. Accurately abstract medical records into current abstracting system.
Responsibilities
Responsibilities:
Reviews all inpatient, emergency, outpatient surgical and ancillary records/accounts for correct assignment of ICD-10/CPT codes utilizing coding books, reference materials, and 3M Encoder. Codes a variety of services utilizing source documents. Queries physicians for clarification on diagnoses utilizing pre-printed coding inquiring forms and letters. Clearly explains questions, resulting in correct documentation and code assignment. Enters medical record abstracted data into the Soft Med system. Works with the billing department to correct coding for rejected claims as appropriate. Maintains knowledge of coding changes through Coding Clinic, seminars and reference materials made available. Achieves and maintains coder certification from a nationally recognized coding association: Certified Professional Coder through AAPC or Certified Coding Associate through AHIMA. Assists the Medical Record Director and Supervisor with coding audits as assigned. Participates in group discussions on coding changes, audits. etc. Maintains knowledge of the DRG and APC system as appropriate to job assignments.

Other information:
CCA 2 years' experience (including the 6 months prior to getting the CCA) or CPC, CCS, RHIT, or RHIA 1 year experience. With Certifications require 2 years' experience, or 1-year experience at a Coder I and successful completion of the Beebe Coder Transition Program. Coder II will code the following services: observation, outpatient surgery, interventional radiology, outpatient cardiac surgical, wound care, emergency and ancillary services. After 2 years ability to go into a Transition program to IP Coder. Successfully scoring over 90% on outside audits and 92% on combined audits.
