POSITION: Patient Care Coordinator (Full Time Regular)
SHIFT: Monday – Friday @ 8am to 5pm
LOCATION: Corporate Office – Gardena, CA
Role and Responsibilities: The Patient Care Coordinator is to support patients and coordinate with clinical staff, care providers, and community resources. Looking for a compassionate professional who believes in facilitating wellness and recovery for patients and their families.
· Will take initiative in instituting care coordination policies and communicating with patients and providers.
· He or she will also assist with enrollment into applicable assistance programs (such as Medi-Cal or Covered CA), preregistering the patient with the health center, and connecting patients to resources.
· Serve as the main point-of-contact for patient intake into case management programs offered by BHS Health Center Network.
· Under the supervision of an RN or medical provider, devise procedures and protocols for patient intake, risk stratification, and filtration so that each enrolled patient receives consistent, attentive care.
· Must be available for consultations for high-acuity patients at any one of the BHS Health Center Network clinic sites or other sites.
· Oversee intake, transfer of care and discharge activities for patients enrolled in specialty case management.
· Provide case management of specialty referrals and linkages to and from community-based partners and other needed services related to a given program.
· Communicate regularly with patients and their families about plans of care and treatment protocols.
· Answer patients’ questions about their care, treatment plans, and all other issues so they feel safe and secure in our care.
· Monitor and adjust patient statuses based on changing needs and conditions.
· Coordinate referrals related to the enrolled program to and from BHS Health Center Network.
· Address any patient concerns with appropriate in-house departments.
· Track all patients enrolled in the case management program.
· Coordinate case conferences with relevant care providers and resources.
· Report all care management activities and outcomes to executive management.
· Maintain and foster relationships with community providers, agencies and organizations involved in care transitions and continuity of care for patients.
· Works collaboratively as a team member with all staff members.
· Promote and believe in BHS mission statement.
· Other duties as assigned.

Required Knowledge, Skills and Abilities
· High School diploma, GED or equivalency.
· Associate’s degree or higher in healthcare or a related field (will consider experience in lieu of degree)
· Possession of current California Driver’s License.
· Ability to relate to the public regardless of racial, ethnic, economic, sexual orientation, and gender identity status.
· Ability to work independently and as part of a team.
· Experience working for a community clinic and/or with Medi-Cal and Medicare.
· Knowledge of drug and alcohol dependency, mental health, family systems and/or 12-Step/recovery resources in the community.
· Ability to demonstrate/convey respect for cultural and lifestyle diversities of clients and staff.
· Ability to present to diverse communities/populations, at all levels of internal/external organizations.
· Ability to recognize personal issues that may have an impact on job performance and interactions with clients.
· Ability to note documentation in electronic medical record system.
· Strong verbal and written communication skills.
· Proven track record of, and ability to demonstrate ethical and professional standards of behavior.
· Basic knowledge of referrals, both in and out of the BHS treatment system.
· Basic knowledge and Experience in utilizing Microsoft Office, specifically MS Word and Excel.
· Ability to speak, understand, and read Spanish, especially Medical Spanish.
Desired Knowledge, Skills and Abilities
· Bachelor’s degree in healthcare or a related field.
· Experience participating in Patient Centered Medical Home (PCMH) model of care.
· Experience working in drug and alcohol dependency or behavioral health.
· Experience developing care plans.
Must have valid California driver’s license and liability insurance if driving personal vehicle on BHS business. Vision, hearing, manual dexterity and eye-hand coordination must be adequate for performance of job duties. Able to sit at desk, use keyboard, write and physically perform other job duties. Able to move about the facility to observe clients and staff.
Recovering individuals must have a minimum two years clean, sober and abstinent to be considered for employment.
