General Job Description:
The Care Coordinator is responsible for service coordination and outreach services to patients and clients of the health center. The Care Coordinator will be responsible for the provision of intensive home visitation services for high-risk families.
Education and Experience Required:
Bachelor’s Degree in applicable field and two years of experience related to social services.
Special Requirements:
Must have current NM Driver’s License and a clear driving record
Must pass a background check
Ability to speak English and Spanish when relevant
Maintains appropriate confidentiality

Essential Job Functions:
Responsible for providing service coordination to patients and clients of the health center including intake interviews, assessment, development of treatment plans, and follows-up in the following areas:
a. Immunization
b. Well-child visits
c. Infant Mental Health
d. Prenatal
e. Health Education
Respond to referrals within the Ben Archer Health Center and other agencies providing services to families.
Interact with providers and other staff of Ben Archer Health Center regarding client health issues to facilitate comprehensive treatment, referrals to other agency programs and provide follow-up for clients.
Responsible for completing training on Medicaid on Site Application Assistance Program.
Responsible for verifying Medicaid eligibility and Social Security information on the NM Medicaid portal for people they enrolled in Medicaid.
Provide promotional services targeted toward families using the health center. Services to include ongoing recruitment & outreach, home visits, and networking & promotion.
Responsible for providing outreach services to patients and clients of the health center including activities which will encourage health care and education, develop community resources and target specific populations to assure existing goals and objectives of the Social Services Department will be met.
Responsible for preparing and documenting all visits in the electronic database within within two business days. Documentation includes intakes, assessments, treatment plans, referrals, etc., monthly reports, and correspondence.
Maintain confidentiality of consumer records & privileged information at all times.
Work with other agencies, groups and organizations to assure development of mutually consistent objectives compatible with existing goals, policies and practice.
Assists patients and client of health center in using effectively developed resource networks.
Participates in staff development, professional meetings, continuing education training conferences and shares information with other staff members.
Supports EEO and PI policies and established standards of Ben Archer Health Center.
Works in a culturally sensitive and safe manner.
Performs other duties as assigned.
