POSITION SUMMARY: Provide outreach, engagement, navigation, education, linkage to resources and support to clients enrolled in the BHcare Adult Clinic. Support the Clinical staff in meeting the comprehensive needs of the clients.
ACCOUNTABLE AND RESPONSIBLE FOR THE FOLLOWING RESULTS:
- Provide limited transportation as needed while clients learn individual transportation skills.
- Support the IOP, Crisis, ACT, and outpatient staff in client outreach to increase adherence and retention.
- Assist clients with connecting to medical and/or psychiatric appointments as needed.
- Provide education, support and consultation to family members, significant others and community members to meet the rehabilitation goal(s) set by the client.
- Develop individualized and generalized community supports.
- Connect clients with needed services and supports (including AA/NA), including making internal referrals when more ancillary support services are needed.
- Assist clients in obtaining, understanding, and maintaining benefits.
- Link clients to community resources including clothing banks, soup kitchens, diaper banks, transportation resources, etc.
- Assist with the management of medication management only clients which may include, but no limited to, paperwork, treatment planning, outreach, and discharge.
- Required to use own vehicle on company business, or to transport clients.
- Attend training sessions, workshops, seminars as requested by supervisor, or as needed for professional development, or as required for accreditation or licensing.
- May be asked to provide case management, psycho-educational services, lead/facilitate family group, co-facilitate groups for IOP, orient new clients to the IOP, and participate in the development and ongoing process of an integrated service plan as program needs dictate.
- May be asked to administer breathalyzer/urine toxicology screens when necessary.
- Perform other duties as assigned.
- Regular and consistent attendance is an essential function of this position.
MINIMUM QUALIFIATIONS
- Bachelors degree required, bachelors degree in social work preferred.
- Knowledge of Psychiatric Rehabilitation is desirable.
- Must have and maintain a valid CT drivers license if relocated to CT as a state resident more than 30 days ago. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organizations auto insurance plan.
- Must be proficient with PCs and Microsoft Office Word & Outlook, and if applicable ability to navigate through electronic health record.
