What are the responsibilities and job description for the LPN-MDS COORD position at Blessing Hospital?

This position is responsible for the implementation of the Minimum Data Set, coordination and completion by all other disciplines of the Resident Assessment Instrument within the time frame specified in the regulations, assisting in the development of the interdisciplinary care plan and maintaining current knowledge of any changes in interpretations of the regulations pertaining to the RAI. All position duties are to be conducted under the supervision of an RN.
This position requires providing services to one or more age populations, including adult through geriatric, in a manner that demonstrates an understanding of the functional/developmental age of the individual served.
This position requires full understanding and active participation in fulfilling the Mission of Blessing Hospital. It is expected that the employee demonstrate behavior consistent with the Core Values while supporting the strategic plan, goals, and direction of the Performance Improvement / Patient Safety Plan at Blessing Hospital.

Graduate of an LPN School of Nursing

Excellent written and verbal communication skills
