Care Navigator Responsibilities:
The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Colorado typically falls between $25.00 - $27.00 hourly with the potential for performance-based bonuses and other benefits. Care Navigator Responsibilities:
- Manage the administrative functions for a team of 200 homebound patients and their providers (Nurse Practitioners or PAs)
- Ensure the care needs of a complex, homebound patient population are met by providing exceptional customer service, coordinating timely visits, and demonstrate superior communication across care teams
- Build trusted relationships with internal and external stakeholders including providers, home health and hospice agencies, patients and families, and Bloom senior leadership
- Own team performance measures that translate to exceptional patient outcomes, including provider productivity, patient satisfaction, and timely answering of calls
- Demonstrate superior time management and prioritization skills by balancing competing demands including referrals, visit, records and supply requests
- Ensure patients are seen in a timely fashion as requested by providers, nurses, and incoming requests
- Build efficient provider schedules to ensure all patients are seen within most effective driving route
- Creating provider schedules
- Answering incoming phone calls
- Confirming appointments
- Scheduling urgent appointments
- Processing incoming electronic faxes
- Processing provider requests for patient records
- Ordering DME for patients as requested by providers
- Ordering oxygen for patients as requested by providers
- Sending referrals to specialists, home health or hospice as requested by providers
- Demonstrating superior communication across care team
- C.N.A license required
- College degree
- Strong leadership skills
- Strong computer skills
- Ability to multi-task at a very high level
- Strong problem-solving abilities
- Excellent phone/customer service skills
- Familiarity with EMR software a plus (eCW preferred)
