A reputable home healthcare provider in Indiana is searching for a Registered Nurse for its Avon territory. In this role, you will average 20 visits per week and manage a range of responsibilities, including assessing patients, establishing care plans with providers, patients, and families, communicating expectations to other healthcare team members, and initiating discharge planning. Responsibilities:
- Deliver direct and compassionate patient care in a home setting, utilizing the standard nursing practice, including assessment, planning, intervention, implementation, and evaluation.
- Develop and implement appropriate care plans based on the specific needs of the patients and families.
- Educate patients, caregivers, and families on how to appropriately manage care.
- Document treatment and progress toward desired outcomes.
- Administer medications to patients per MDs orders.
Qualifications:
- An ADN is required, and a BSN is preferred.
- Active RN license.
- 1 years of acute care nursing experience is preferred.
- Home health nursing experience is a plus.
Additional info:
- Full-time, permanent position.
- Schedule: Monday-Friday, 8a-5p 1 Saturday shift every 6 weeks.
- Pay $34-38/hr., depending on experience bonuses
