What are the responsibilities and job description for the Day Habilitation Specialist position at BRHC?
As the Day Habilitation Specialist, you will act as a member of an inter-disciplinary team of professionals to assess and develop pre-vocational, vocational and day habilitation programs for the I/DD individuals being served in the program. These programs will be person centered, individualized and focused on developing an environment of success for each individual to reach their highest level of competency with respect to working in the community or in a self-directed business setting. This will include but not be limited to social skills training, pre-employment training, pre-vocational and vocational training, environmental safety, volunteering, personal hygiene, community inclusion, business etiquette and so much more.
SHIFT: Monday-Friday8:30am-4:30pm
ESSENTIAL DUTIES:
· Resourcing Community Opportunities
· Provide services, as identified in Individualized Service Plan (ISP). Areas may include, but are not limited to: Personal Hygiene, Clothing Care, Transportation, Medication Administration, Money Management, Recreation, Community Inclusion, Meal Planning and Preparation, Social Skills, Communication, Personal Safety
· Assist individuals who wish to participate in community activities.
· Communicate effectively with the individual’s family members.
· Provide a safe and therapeutic environment and promote self-advocacy.
· Assist with medical appointments.
· Administer prescribed medications and/or first aid.
· Participate in scheduled support team meetings.
· Assist in developing ISP strategies and complete daily activity logs and behavioral data.
· Attend agency in-service training.
· Participate in the consumer's interdisciplinary team meeting.
· Cooperate with the Agency and Department staff in any inspection or investigation.
· Provide transportation in agency vehicles.
· Performs any other duties as requested by the Company
