What are the responsibilities and job description for the Patient Care Coordinator position at Brightmoor Hospice?
The Patient Care Coordinator is responsible for daily referral coordination and patient intake process for Hospice Services. Patient Care Coordinators work closely with Clinical Management and Case Management team to complete the intake and referral process.
Skills and Qualifications:

- Knowledge of relevant software applications: word processing, spreadsheet, and data base management.
- Knowledge of spelling, punctuation, grammar, and other English language skills.
- Knowledge of administrative and clerical skills.
JOB DUTIES:
1. Assists with updating weekly reports.
2. Provide clerical/administrative support.
3. Prepare responses to correspondence containing routine inquiries.
4. Perform general office duties such as ordering supplies, maintaining records management systems, and copy machine key operator.
5. Coordinate and direct office services, such as personnel in order to aid office staff.
6. Prepare invoices, reports, memos, letters, and other documents, using MS Office spreadsheet, budget available resources, and copy machine key operator.
7. Answer phones, take messages, sign for packages, deliveries and run errands.
8. Displays a willingness to support policies and procedures and uses appropriate channels for changes in policies.
9. Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation and job performance.
10. Accepts responsibility for regular attendance and punctuality.
11. Read and analyze incoming memos, submissions, and reports in order to determine their significance and plan their distribution in admissions and referrals.
12. Keeping track of patient’s certification, recertification and IDT’S.
13. Preparing for IDT.
14. Attending IDT and assist the physician.
15. Filing
16. Open IDT orders and recertification orders
17. Close IDT orders after all disciplines have documented.
18. Serves as a resource person for all employees.
19. Other assigned duties.
