Job Summary:
The Community Care RN has the responsibility for providing direct and indirect nursing care to PACE participants. Works under the direct and indirect supervision of the Community RN Supervisor or designee but is independent in the application of advanced nursing knowledge and skills. Manages complex clinical situations for which they are responsible.
Duties will include collect patient data and completes required forms; Identified participant's overt problems/needs and set priorities; Completes initial and annual assessments; Develops plan of care, communicates changes; Provides home visits, medication management, and medication education to participants and/or family as needed; Adheres to work flows for clinic operations; Attends meetings as assigned; Demonstrates active participation in Q1 processes.
- Must be self-directed, flexible and committed to the team concept.
- Able to read, write, speak, and understand the English language.
- Possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
- Function independently and have flexibility, personal integrity, and the ability to work effectively with older adults, personnel, and support organizations

Required Qualification:
• CPR Certification
• State of Michigan driver's license and maintain an acceptable driving record
• Reliable transportation
• Minimum of 1-year experience with the frail and elderly or long-term care population.
• Licensed Registered Nurse in the State of Michigan
• Graduate of an accredited School of Nursing
