***Must hold a valid AZ Caregiver Certification***

Job Description

Our Care Coordinator work closely under the Health and Wellness Director to plan resident care. You will oversee operations at the community and monitor resident health routines. Some of your duties will include overseeing health care administration and staff management, as well as to make sure staff members are following health care guidelines.

Responsibilities

• Hire, train, lead and mentor Caregivers
• Assure team is in compliance with regulations governing resident care
• Create schedules and approve timecards
• Complete assessments, assist with incident follow ups and any tasks related to resident care
• Rotating on call of every other week, primarily phone based but occasionally required to work on the floor. 

Qualifications

• Previous experience of at least 2 years as a caregiver in Assisted Living
• Management/ Leadership experience of at least 2 years preferred
• Scheduling experience strongly preferred
• High School Diploma or equivalent.
