Salary Range: $25.00-30.00/hour (DOE) (Anticipated hiring range $25.00-28.00/hour)

Summary:
The Intake Coordinator is responsible for facilitating patient care by assessing and evaluating patient’s physical, psychosocial and emotional needs. The Intake Coordinator helps support and coordinate patient access, appointments and medical information and is the primary clinical contact which functions as a Navigator and Patient Advocate between patient, their family and the care team.
Key Responsibilities:
- Review medical records for individuals to establish candidates for Proton Therapy treatment
- Inform patients of additional testing and/or treatments required
- Receive, track and report on all physician referrals for consultation and/or treatment
- Monitor care and data to optimize patient outcomes including collaboration with medical staff to facilitate optimal patient care
- Facilitate the education of patients and families. Acts as a resource to health care team
- Act on behalf of the patients and family to ensure care is safe and appropriate with consideration for values, diversity and human rights
- Utilize organizational skills to assure continuity of care in collaboration with other disciplines through appropriate planning, education, and coordination of other services
- Coordinate with interdisciplinary team members to gather all necessary data for consultations
- Coordinates the delivery of safe and appropriate patient care governed by evidence-based clinical practice
- Maintain records, prepare reports and compose legible correspondence/documentation on patient record of progress notes, telephone calls, patient education, instructions and unusual situations
- Serve as resource for information about services provided for inquiries from physician offices, potential patients and families from outside San Diego County.

Qualifications/Experience/Education:
- Certified Medical Assistant strongly preferred or equivalent education and experience in oncology.
- Bachelor's degree preferred
- Minimum of 3 years of oncology experience in a hospital or clinical setting.
