Job Summary
Under the supervision of the Director of Nursing or delegate, coordinate all medical referral tracking for agency patients and their families; assist with performing clerical duties, medical translation, and coordinating referral gaps for all localities within the agency to close the referral loop. Attends all external meetings relating to this scope of practice and updates the agency accordingly.
Primary Duties and Responsibilities
§ Monitor and regularly update Referral Tracking Log/System
§ Logs all referrals, based on status of referral determines when to follow up.
For STAT referrals scheduled within 24 hours, report received within 72 hours.
For Urgent referrals scheduled 24-72 hours, report received within 1 week.
For Routine referrals scheduled 72 hours-2 weeks, report received within 30 days. Documents in the log when the consult report is received.
§ Follow up with the specialist’s office when a consultation report is not received.
§ Schedules return appointments for all patients who have been referred to specialist. This will not only ensure the patient has seen the specialist but also give the provider an opportunity to discuss the findings with the patient.
§ Document in structure data as appropriate to ensure mapping of referral
§ Cross-trains team members so that system is reliable and efficient
§ Submit referral tracking activity reports as assigned by Director of Nursing
§ Performs duties with regard to agency mission statement and standards of practice
§ Performs required clerical work relating to patient care
§ Demonstrates customer service skills while answering office telephones for the medical department or while making patient referrals to outside agencies
§ Initiates all agency and/or patient referral tracking per agency policies
§ Assist with routing intra-office as well as external fax reports
§ Maintains and ensures confidentiality per HIPAA and agency guidelines
§ Retrieves medical records from the agency’s electronic medical record database
§ Maintains medical records in an efficient manner
§ Participates in economical utilization of supplies and equipment and submits cost effective orders
§ Validates all patient data prior to completing referrals
§ Explains the medical center’s referral system to all patients
§ Provides patient education relating to referral compliance
§ Performs other duties as assigned

§ Must be computer literate with ability to demonstrate typing speed of at least 30wpm
