Job Mission / Reason to Be.
Provides personal care to patients. This position typically travels to a patient's home and help them perform basic tasks like cleaning the house, bathing the client and providing them with companionship and emotional support. QMAP may also be responsible for driving and escorting clients to appointments, grocery trips and other important errands. They may also help the client with any mobility issues, like helping them in and out of their beds or chairs and pushing them in wheelchairs. Strong emphasis on all aspects relating to that of a community activity specialist.

Main Responsibilities
- Serving as a companion to clients
- Transporting clients to and from appointments, errands, activities, etc.
- Managing medication
- Preparing meals
- Interpersonal skills, including exceptional patience and listening skills
- The ability to provide care across a variety of settings, including group homes and day service programs
- Implement process standards and policies
- Manage Daily client activities in the community
- Take individuals in the community for daily activities and maintain daily log
- Troubleshoot client issues while in the community
- Planning and monitoring the day-to-day running of business to ensure customer satisfaction
- Develops and implements growth strategies for acquiring and approving new clients
- Paid holidays, vacation time and sick leave.
Rewards
