Job Title: Long Term Care (LTC) Geriatric LPN Job Summary:We are seeking a qualified Licensed Practical Nurse (LPN) to join our team at a Long Term Care (LTC) facility specializing in geriatric care. The LPN will be responsible for providing direct patient care, administering medications, and implementing individualized care plans for elderly residents. Responsibilities and Duties:1. Provide nursing care in accordance with facility policies and procedures2. Administer medications and treatments as prescribed by physicians3. Monitor residents' health status and report any changes to the nursing team4. Assist with activities of daily living, including bathing, dressing, and feeding5. Collaborate with the interdisciplinary team to develop and implement care plans6. Educate residents and families on proper care techniques and health maintenance7. Maintain accurate and up-to-date medical records8. Follow infection control guidelines to ensure a safe and clean environment for residentsQualifications and Skills:1. Current LPN license in the state of employment 2. Minimum of 1 year of experience in long term care or geriatric nursing3. Strong communication and interpersonal skills4.
