Position Summary: Our DCW team implements approved behavior plans that are developed by our clinical team. Must be physically able and willing to provide intimate personal care to residents in-home and in the community as needed (may include but is not limited to lifting, transferring, toileting, feeding, bathing, and dressing). Is responsible for transporting residents to any activities, visits or events if needed as well as actively participating in these events, documents daily behavior, unusual incidents, accidents, and any other information that pertains to each individual care. The DCW provides vital information for the development and modification of individualized service plans (ISP) and assists and models a variety of supports (may include, but is not limited to appropriate nutrition, personal hygiene, life, and social skills, etc.). Must ensure the accurate completion of required documentation and administration of medication and completes designated shift responsibilities. Direct Care Workers must attend all required training and meetings that may occur outside of their regular shift.

Qualifications/Requirements:

  • Must be at least 18 years of age;
  • Completion of training from the College of Direct Support required.
  • Minimum of a High School diploma with prior experience working with children and adults with developmental disabilities and/or behaviorally challenged individuals;
  • Valid driver's license in the state of NJ if willing to commute with a client. (Not mandatory)
  • Good English writing and communication skills;
  • Employee must successfully complete and demonstrate proficiency in all areas of required training;
  • Ability to effectively work with people of diverse cultural backgrounds and races;
  • Ability to effectively work as a team member and implement positive behavioral supports;
  • Complete drug test and background checks including finger printing.
  • Must cooperate with the licensee, Department of Human Services (DHS) staff, Department of Children and Families (DCF) staff and all state representatives in any inspection or investigation.
