What are the responsibilities and job description for the Plan Writer position at CARES?
The mission of CARES is to assist children and adults with intellectual/developmental disabilities (ID/DD) and/or mental health conditions and their families, in particular, from under-served multilingual communities, access information, obtain resources and services.
Salary Range: $50-$55k Location: Bensonhurst, NY additional travel required
Essential Duties and Responsibilities:
Create and execute service plans for HCBS clients. This includes conducting parent/child meetings and writing a comprehensive service plan within 48 hours of the scheduled meeting.
- Collaborate with all parties necessary to complete service plan (referral service, staff, parents etc.).
- All tasks are completed within 48 hours of plan meetings.
- Maintain compliance with all company policies and procedures.
- Respond to supervisor feedback on related tasks.
Education and/or Work Experience Requirements:
- Excellent verbal and written communication skills, including ability to effectively communicate with internal and external customers
- Computer proficiency (MS Office Word, Excel and Outlook)
- Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.
- Experience in technical writing is a plus.
- High school diploma or GED required, Associates degree preferred.
Physical Requirements:
- Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
- Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
- Must be able to lift and carry up to 35 lbs.
- Must be able to talk, listen and speak clearly.
Salary : $50,000 - $55,000
