Key Responsibilities:
- Building and nurturing relationships with clients and their families to ensure exceptional service and support.
- Coordinating care plans and schedules, matching clients with compatible caregivers, and conducting regular follow-ups.
- Collaborating with healthcare professionals, including doctors, therapists, and social workers, to develop and maintain comprehensive care plans.
- Overseeing client Authorization their changing needs and preferences.
- Addressing and resolving client concerns, ensuring satisfaction and seamless service delivery.
- Identifying opportunities for improvement, implementing new policies and procedures as needed.
- Staffing Open Cases
- Ensuring compliance with federal, state, and local regulations, as well as maintaining up-to-date knowledge of industry trends.

Pay: $22.00 per hour
Expected hours: 40 per week
