At CARESPHERE www.carespherehc.com, we are in search of a dedicated and enthusiastic Home Care Client Coordinator to become a key member of our thriving team. Our mission is to provide top-notch, personalized care to our clients, and we need your talent and passion to help us achieve that.

Key Responsibilities:

  • Building and nurturing relationships with clients and their families to ensure exceptional service and support.
  • Coordinating care plans and schedules, matching clients with compatible caregivers, and conducting regular follow-ups.
  • Collaborating with healthcare professionals, including doctors, therapists, and social workers, to develop and maintain comprehensive care plans.
  • Overseeing client Authorization their changing needs and preferences.
  • Addressing and resolving client concerns, ensuring satisfaction and seamless service delivery.
  • Identifying opportunities for improvement, implementing new policies and procedures as needed.
  • Staffing Open Cases
  • Ensuring compliance with federal, state, and local regulations, as well as maintaining up-to-date knowledge of industry trends.

Job Type: Full-time

Pay: $22.00 per hour

Expected hours: 40 per week
