Our main goal is to help primary care physicians monitor patient care plan adherence as well as proactively identify and escalate earlier interventions in-between appointments. Caretrack is seeking a talented Clinical Case Worker to join our team. The successful candidate will be responsible for providing quality care gas assessments for our patients, collaborating with healthcare providers and clinical staff to ensure patient adherence to their care plan. As a Clinical Case Manager, you will utilize your clinical skills and experience to support the development of a high-performing culture and work with team members to close care gaps using client care plan assessments.

Registered Nurses or Licensed Practical Nurse required.

Responsibilities

  • Conduct initial patient care gap analysis and complete client care-plan assessments
  • Create (re)assessment of physical, mental, cognitive, psychosocial, functional and environmental and inventory the resources and supports; a comprehensive care plan for all health issues, which should be revised as necessary;
  • Provide information and refer to resource services
  • Identify problems, eligibility for assistance, and need for services
  • Coordinate other care providers such as primary care and specialty physicians, to avoid unnecessary costs and miscommunications
  • Provide medication management oversight, telephone check-in service
  • Attend and participate in scheduled meetings with fellow staff members
  • Work as part of a team of Care Coordinators, Case Managers and to extend advice on cases when needed
  • Support and educate clients, their care givers and related parties on issues related to the client
  • Ability to work a flexible schedule
  • Maintain and update client files and complete required documentation
  • Maintain accurate time records, calculate and report hours worked with clients
  • Support and endorse CareTrack’s philosophy, programs and culture
