Job Summary
The Community Support Services Direct Care Staff member’s duties include: (1) Contribution to the development, implementation, monitoring, and updating of the individualized rehabilitation plan pursuant to N.J.A.C. 10:37B-2.4. (2) Perform therapeutic rehabilitative skill development pursuant to N.J.A.C. 10:37B-4.6. (3) Offer services appropriate to meet the consumer’s needs pursuant to N.J.A.C. 10:37B-4.6. (4) Perform crisis intervention pursuant to N.J.A.C. 10:37B-4(a). (5)Coordinate and manage services that assists the consumer in reaching their individualized rehabilitation plan goals and objectives.
Minimum Education/Certifications & Work Experience
Bachelor's Degree from an accredited college or university in one of the helping professions, such as social work, human services, counseling, psychiatric rehabilitation, psychology, or criminal justice. Staff with a Bachelor’s level degree in a field other than a helping profession shall have a minimum of two years of professional experience in a community behavioral health setting. Certification as a psychiatric rehabilitation practitioner may be substituted for one year of experience. Staff employed under this subsection shall work under the supervision of a physician, psychiatrist, licensed psychologist, advanced practice nurse, registered nurse, or master’s level community support worker and shall deliver the services set forth in (1) through (5) above, provided that staff is authorized to perform that service under their professional licensure, certification, or credentialing standards. Must have a valid New Jersey Driver’s License in good standing according to NJ official driver’s abstract as the position may be asked to transport consumers.
Position Type/Expected Hours of Work: Must be willing to work holidays, weekends and provide additional coverage as needed
Working Conditions: Requires full range of body motion including handling and lifting small items. Occasionally lifts and carries items weighing up to 50 lbs. Requires corrected vision and hearing to normal range. Requires working under stressful conditions and flexible hours. Involves some discretion, independent action and judgment to decide what to do.
