What are the responsibilities and job description for the RN - Patient Advisory Nurse position at CARLE?
Deliver telephone nursing care for patients of Carle Foundation Hospital and its affiliates, Health Alliance, and other contracted services. Using nursing processes to assess, evaluate, and prioritize patient calls regarding symptoms and/or other medical concerns. Provide clinically appropriate direction to manage symptoms and treatments based on the presented health concern within established guidelines and clinical judgement for urgency of intervention.
- Acts as preceptor and supports the development of other staff and formal learners.
- Documents accurately and completely.
- Performs health assessments via telephone using Adult Telephone Protocols Office version 3rd edition.
- Provides education and counseling to patients, families and groups.
- Supports maintenance and completeness of patient's electronic medical record, including medication reconciliation.
- Primarily provides telehealth triage for acute needs of ambulatory patient population for the Carle organization and community at large as well as contracted services to additional health systems and Health Alliance.
- Provide initial and transitional support to patients across the Carle care continuum to support critical findings and alerts, provide after-hour support for critical patient communications, and facilitate delivery of care.
- Assist with nursing in-baskets (MRC, Nurse Advice, etc.) as business and patient needs dictate.
