GENERAL RESPONSIBILITIES:
1. Develop and maintain a cooperative working relationship with service providers and organizations offering supportive services to older adults to ensure prompt service implementation and adjustments in response to changes in the client’s status.
2. Prioritize client concerns and unmet needs, as well as, the necessary documentation to address all issues in a timely manner.
3. Complete identified community education activities as assigned.
4. Attend staff meetings, in-services, and conferences as recommended by supervisor.
5. Possess the ability to work independently bringing concerns or challenges regarding any issues to the attention of direct supervisor.
6. Other duties as assigned within the guidelines of this position.
ESSENTIAL FUNCTIONS:
1. Acquire healthy aging certification(s) (such as Matter of Balance or Chronic Disease Self-Management).
2. Provide active intervention and advocacy on behalf of the client to access necessary services from state, federal, and local programs and to resolve problems experienced by the client.
3. Complete at minimum quarterly monitoring of clients by phone or in person according to individual client needs.
4. Create and maintain confidential case records including, documentation of follow-up and termination.
5. Meet all Illinois Department on Aging & Title III rules.
Requirements
1. A high school diploma or equivalent is required for this position. Must be working toward obtaining a degree in a program with preferences in the areas of social work, health, social sciences, health care administration, gerontology or criminal justice or related field. All degrees must come from a regionally accredited educational institution which has U.S. Department of Education approval.
2. Enrolled in an accredited college or university social work or related program that supports and requires a field internship.
3. Ability to relate well to persons with disabilities and older individuals respecting their privacy, dignity, and right to self-determination.
4. Must demonstrate fundamental communication and problem solving skills.
5. Ability to work independently.
6. Ability to work effectively and constructively with persons from all ethnic, racial, and social backgrounds.
7. Ability to work effectively with co-workers and other community agencies.
8. Proficiency in the use of Microsoft Office applications including Outlook, Word, and Teams.
9. Strong verbal and written communications skills.
10. Passage of complete background clearance, physical and Tuberculosis (TB) Test.
11. As an employee of the Agency you are expected to support these codes to the extent doing so is consistent with and allowed by applicable law.
12. A valid driver’s license, reliable means of transportation and proof of liability insurance is required.
