What are the responsibilities and job description for the Long Term Care Ombudsman Coordinator-FT position at Catholic Charities of the Diocese of Albany?
We are currently hiring a Long Term Care Ombudsman, to work in our Tri-County Services office. As the Long Term Care Ombudsman, you would be responsible for:
-Represent the Ombudsman program by reviewing and responding to resident, family and/or community members complaints regarding LTC facilities, adhering to the highest level of confidentiality.
-Meet with residents and family members, conduct investigations around complaints, and interview facility staff
-Effectively manage and prioritize complaints received, ensure a timely and relevant response to all complaints assigned, using conflict resolution techniques and strategies
-Advocate for resident rights including avoidance of elder abuse, mistreatment, and neglect.
-Liaison with resident councils in all LTC facilities, provides presentations for staff, residents and community regarding Ombudsman services, resident rights, and other pertinent topics.
-Enter data into databases accurately and timely, and report to Program Director on outcomes, trends, and activities.
Support a corps of volunteer ombudsmen through providing training and guidance in complaint handling.
Long Term Care Ombudsman, Minimum Salary: $25.00 per hour
Long Term Care Ombudsman, Maximum Salary: $25.00 per hour
Long Term Care Ombudsman, Schedule: Mon-Fri 35 hours per week (full time)
Qualifications:
-Bachelor’s Degree in social work, human services, health, public administration, gerontology, law, or related fields
-A valid driver’s license and maintain required vehicle insurance with access to a vehicle during work hours
-Experience working with older adults in a long-term care facility or as an advocate is strongly preferred
-Working knowledge of Microsoft Office software and other computer applications
