As the Bridge Clinic Coordinator, you will play a pivotal role in individuals' lives, making a significant impact through:

• Coordinate patient recruitment, referrals, intake and plans of care for individuals receiving services 

• Facilitate linkage to maintenance medication providers and other supports as indicated
• As needed, provide direct client services including intake, assessment, and case management services
• Utilize agency and contract specific systems and data to maintain client files
• Attend community trainings and workshops
• Coordinate service provision with medical providers

Bridge Clinic Coordinator Minimum Salary: $58,500.00 annually

Bridge Clinic Coordinator Maximum Salary: $58,500.00 annually

Bridge Clinic Coordinator Schedule: 8:30 – 4:30, Monday - Friday

Qualifications:

• Associate’s Degree in Health, Education, Human Services or a related field
• 1-3 years of experience
• Experience working with diverse populations
• Ability to maintain strict confidentiality
