Central City Concern is an innovative nonprofit agency providing comprehensive services to single adults and families in the Portland metro area who are impacted by homelessness, poverty and addictions. Located in the heart of the Old Town Chinatown neighborhood, Old Town Clinic supports compassionate providers and treatment for acute and chronic health concerns, primary care, mental health support, care for substance use disorders and an on-site pharmacy.

The Behavioral Health Services Coordinator (BHSC) works collaboratively among the Patient Centered Primary Care Home (PCPCH) multi-disciplinary teams to coordinate integrated behavioral health services and patient resource access for Central City Concern primary care patients. They are responsible for advocating and supporting the behavioral health needs of clients of Old Town Clinic including providing clinic support and care coordination.

Under the supervision of the Program Manager II, BH, the BHSC will plan, develop, and implement protocols, programming, and workflows for the purpose of meeting short-term or long-term goals as established by CCC and Primary Care leadership. The BHSC works closely with partner programs within Central City Concern and in the community to improve health outcomes and patient experience as part of a comprehensive approach to ending homelessness and achieving self-sufficiency. The BHSC will support primary care mental health (MH) services including those provided by PMHNP’s and BHC’s. The BHSC will engage in revenue generation by conducting wellness groups and providing other clinically appropriate services.

Schedule: Monday - Friday

Location: Old Town Clinic (727 W Burnside Street, Portland, OR 97209)

Compensation: starting at $22.24 per hour, rate could increase depending on candidate's experience. 

MINIMUM QUALIFICATIONS

  • Education and experience sufficient to succeed, carry out responsibilities, and demonstrate the skills and abilities as described above with at least two years’ experience working in a clinical setting. BA/BS in behavioral health/social services or equivalent health care experience preferred. Experience working in mental health field preferred.
  • Must have an understanding of the causes of homelessness, the medical and psychosocial context of homelessness, and the systems and processes used to help clients secure and maintain health and self-sufficiency.
  • Must have current CPR certification prior to start date.
  • Must have a working knowledge of medical terminology.
  • Must be able to be trained in the use of EHR.
  • Must have completed non-violent de-escalation training or complete within 60 days of hire.
  • Must have a high tolerance for ambiguity and change.
  • Knowledge of the network of services available to homeless persons, and at least one year experience working with homeless persons preferable.
  • Bilingual Spanish/English preferred, but not required.
  • Significant experience working with high-risk clients in a trauma-informed approach to care strongly preferred.
  • If in recovery, must be able to document continuous abstinence under independent living conditions or recovery housing for the immediate past two years.
  • Must have the physical ability to bend, stoop, twist, kneel, reach, squat, pull and lift heavy clients or objects.
  • Must be able to climb stairs several times a day.
  • Must be able to work efficiently in a very fast paced environment.
  • Must pass a pre-employment drug screen, TB test, and background check to work at Central City Concern and to obtain ID-Badge and/or records access at partner hospitals.
  • Must adhere to Central City Concern’s non-discrimination policies.
  • Must have ability to effectively interact with co-workers and clients with diverse ethnic backgrounds, religious views, political affiliation, cultural backgrounds, life-styles, and sexual orientations and treat each individual with respect and dignity.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Work collaboratively with primary care teams, pharmacy and other clinic staff as well as other Central City Concern programs to continuously enhance integrated behavioral health services in primary care.
  • Organize and facilitate clinical and operational team meetings
  • Provide client services including outreach calls, availability for warm handoffs and ongoing coordination.
  • Coordinate implementation of workflows for specific projects related to behavioral health in primary care.
  • Provide clinical services within the scope of training and qualification, providing accurate and timely documentation and billing codes for reimbursement.
  • Provide service coordination and linkage as needed, including benefits, housing, medical care and behavioral health treatment, employment services, culturally specific resources, peer support, and all other relevant services.
  • Provide training and education for staff and academic learners regarding program as needed.
  • Support primary care clinic regarding behavioral health referrals, transitions of care and coordination with other behavioral health providers and community partners.
  • Follow Policies and Procedures of Old Town Clinic and Central City Concern Health Services.
  • Review and complete documentation within electronic health records (EHR) in a timely manner for relevant patient interactions, appointments and encounters.
  • Participate in compliance, quality assurance and quality improvement activities as directed.
  • Work in concert with Central City Concern’s Complex Care Management programs to support continuity of care and improve patient health outcomes on multiple domains.
  • Attend all mandatory CCC trainings in a timely manner.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.

  • Ability to work courteously and effectively communicate with the general public, patients, medical personnel, social service personnel, corrections personnel, police, merchants, contractors, and co-workers.
  • Ability to function as a team member, be flexible in attitude, and relate effectively with Old Town Clinic patients.
  • Ability to maintain accurate records and necessary paperwork.
  • Ability to communicate clearly and concisely both orally and in writing.
  • Ability to learn and apply training instruction.
  • Ability to perform simple arithmetic computations.
  • Proficiency with Microsoft Office applications, including creating and edit Word documents; creating and working with Excel spreadsheets; and creating, editing, and presenting PowerPoint presentations (or equivalent platforms appropriate to current technology use).
  • Ability to maintain highest standards of confidentiality with regard to patient information.
  • Ability to maintain a calm professional demeanor.
  • Ability to make independent judgment.
