What are the responsibilities and job description for the Care Coordinator Bilingual (English/Spanish) - Full Time position at CCM Navigator?
Care Coordinators (CC) utilize CareTrack's chronic care model to provide care management and care coordination for senior patients. Services provided are not face to face and phone skills are a critical part of the position. CCs work in a climate that allows direct communication between the Care Coordinators, CareTrack Management, medical practices, the client, and appropriate service personnel, in order to optimize the outcome for all concerned.
Care Coordinators complete regular assessments of patient’s care plans. This position compiles, reviews, monitors patient care plans and functional changes of clients. CCs provide patient self-management support and patient education. Assure that patients receive needed evidence-based services and complete, accurate and timely information is maintained regarding patient diagnoses, patient assessment, care relationships and care plans. CCs advocates for clients, collaborates with community service agencies, and provide educational information to Patients. CCs desire to improve each patient’s situation by understanding their individual circumstances with the goal of maintaining or restoring them to sustained health improvement.
Your typical work day involves computer and phone interaction with patients, which requires sitting for long periods of time. Typical Monday-Friday schedule with major holidays off. Saturday option for students/ part time staff when need is available.
Job Type: Part Time that could quickly turn into Full-Time for the right applicant.
