OVERVIEW:
Provides nutrition services to individuals, consultation, and any required clinical supervision related to the provision of nutrition services, functions as an Interdisciplinary Treatment Team (ITT) member, and Ensures compliance with all applicable standards.
RESPONSIBILITIES:
- Provides initial and annual nutrition assessments on all individuals.
- Provides supervision for the dietary technician(s).
- Participates/consults in developing outcomes and methods to address nutritional problems or needs in order to maximize assets or strengths for inclusion in the person’s IPP.
- Ensures entry of all prescriptions for modified or special diets in the individual’s record.
- Regularly reviews, with documentation in the individual’s record, all modified or special diets and weights.
- Provides dietary counseling, education, and consultation regarding nutrition and diet control measures to the individual, staff, family, or personnel, as necessary.
- Ensures the provision of nutritionally adequate meals or nourishment, including adherence to prescribed diets.
- Ensures that the sanitary conditions are in conformity with state and local regulations.
- Ensures that all purchasing, storage, preparation, and serving of food is in conformance with OPWDD policies and procedures and all applicable state and local codes.
- Fulfills all record-keeping and documentation requirements, including at least progress notes, assessments, outcome performance records, for all individuals and review reports.
- Checks individual’s food and beverages served during a meal to ensure adequate nutritional, quality, and quantity periodically.
- Provides or Ensures required or requested education and training to fulfill the food and nutritional needs as well as the use of proper eating skills, techniques, and eating positions of all individuals in conjunction with the DT, SLP, OT, and PT.
- Evaluates and Ensures the adequacy of dining areas, tables, chairs, eating utensils, dishes, and any needs for special equipment to meet individual needs in conjunction with the DT, SLP, OT, and PT.
- Approves individual’s menus and Ensures menus are available to staff, including those for modified or special diets for each daily meal as assigned.
- Attends individual’s reviews and clinical meetings when nutritional input is required or requested by the ITT.
- Adheres to all agency and program policies and procedures.
- Performs all other necessary duties relevant to the position as requested by supervisor.
- Advises direct supervisor of any obstacles to achieving responsibilities.
- Completes annual agency wide trainings on food safety/sanitation, and basic nutrition principles.
- Assists in nutrition/food related policy creation and updates, annually or as needed.
- Assists in ensuring adequate education of staff and individuals of food and fluid modifications, per OPWDD regulations.
- Assists with QA and BPC audits, as needed.
ESSENTIAL ACADEMIC AND EXPERIENCE REQUIREMENTS:
- Registered Dietitian, or Registered Eligible. CDN preferred.
- Must be at least 18 years of age.
- Reliable transportation (NYS Driver’s License required).
- Ability to work independently and motivate others.
- Ability to communicate effectively, both orally and in writing.
- Ability to lift at least 50 lbs. dead weight.
- Maintain all required certifications/training by federal regulations and CDS Monarch policy.

- Attends mandatory education and training as scheduled.
- Obtains and maintains required certification(s).

- Performs all other duties relevant to the position as assigned by supervisor.
