Responsibilities:
-Our Integrated Health Care Coordinators provide:
· Comprehensive Care Management
· Care Coordination
· Health Promotion
· Comprehensive Transitional Care
· Individual and Family Services
Referral to Family and Social Support Services
Qualifications:
Required - Bachelor’s degree in human services field. Understanding of and experience with Serious Mental Illness and/or Serious Emotional Disturbance.
Preferred – 1-3 years experience in Human Services field. Experience navigating MCO portals and processes. Experience with Integrated Health Home services. Working knowledge of Spanish, Burmese or Arabic.-

Pay: $20.00 - $21.00 per hour
Expected hours: 37.50 per week
