Job Description: Join a Team expanding to a location in Indianapolis. Centerstone is opening a new office and is seeking bachelor's level staff to meet the needs of persons with substance use disorders and serious mental illness; community and office based. The Care Coordinator's responsibilities will include engagement, assessment, coordination of care and coaching/life skills trainings for adults, as part of a larger team of therapist and coaches. The Teams focus will initially include transition/expansion of criminal justice partnerships with expansion to serve a broader population. Engagement, crisis, and day to day service delivery and oversight of the client's care are the primary responsibilities. Essential Duties and Responsibilities: Supervisory Responsibilities for: This title does not supervise others. Facilitate client’s use of family, social and community resources, acting as an advocate when necessary and maintaining necessary communication with other involved parties. Assess and develop person-centered plan and monitor progress of the plan. Clinical documentation is accurate, meets organizations standards in composition and content, and is adequate for communication about client’s treatment and progress. Regular and reliable attendance is required. Key Performance Indicators Spend 60% of hours worked providing direct client care Complete ANSA/CANS assessments and reassessments on time. Complete clinical documentation within 24 hours of the service provided. Other Duties and Responsibilities: All other duties as assigned. Job Qualifications: Bachelors degree from a four-year college or university preferred; preferably in psychology, social work, or a related field. Experience working and/or volunteering within the behavioral health, criminal justice, or social services field can be considered in place of degree.
