Cardiology – CNS – St. Cloud, MN
CentraCare is seeking a Clinical Nurse Specialist to join our team of Physicians and Providers working for our award-winning Heart & Vascular Center (CCHVC).
In this role you will be part of the Heart & Vascular team; a highly collaborative team of physicians, providers and multidisciplinary staff working to provide patient and family- centered care throughout the practice.
- Full time, 1.0 FTE
- M-F Days – 8am – 5pm
- No weekends, no call, no holidays
In this position a typical day may include:
- Facilitates daily rounds with providers/staff.
- Reviews complex patient situations and completes follow up as needed.
- Completes SOC- Standard of Care reviews for RL solutions.
- Facilitates and participates in RCA follow up and leads practice changes at the unit, hospital and system levels as appropriate.
- Bases practice updates on complete literature reviews to assure current EBP is in place.
- Manages all order set updates and facilitates necessary changes when needed.
- Champions monitoring and follow up with KPIs- CAUTI, CLABSI, VAP, HAPI etc.
- Leads unit and hospital-based committees and serves as a liaison to regional sites.
Must be certified as a Clinical Nurse Specialist and be eligible for MN licensure. Open to new grads.
