What are the responsibilities and job description for the Low Intensity Care Coordinator position at Central Alabama Wellness?
Travel will be necessary, the case load will be split between our Clanton and Pelham locations.
I. Title: Low Intensity Care Coordinator
II. Classification: Intensive Care Coordinator
III. Supervisor: C&A Division Director
V. Minimum Qualifications: Bachelors Degree, preferably in a Social Science field, a valid Alabama Drivers License, Maintenance of a good driving record, Personal Automobile Liability coverage ($20,000 - $40,000), and completion of Department of Mental Health approved Intensive Care Coordination certification.
VI. Primary job functions and performance expectations:
1. Provides Care Coordination services to the seriously emotional disturbance population.
A. Serves as liaison with other agencies (i.e., local hospitals, DHR, etc.) in identification and coordination of services for SED referrals.
B. Accomplishes initial contact with clients within one week of referral.
C. Develops and implements service plans, which are individualized and directed toward clients' needs.
D. Provides direct case management services including service linkage, crisis intervention, service arrangement, advocacy, needs assessment, providing services in the home, transportation and transportation management.
E. Accomplishes regular progress assessment, service evaluation, and treatment plans in accordance with DMH Standards.
F. Completes all statistical, administrative and clinical documentation within one working day of all pertinent clinical activities.
G. Submits documentation which is concise, accurate, and in accordance with Center policies and DMH Standards.
H. Exercises proper judgment as to when and where clients are referred.
I. Meets assigned quarterly average for billable services (100 hours per month).
J. Maintains and provides accurate consultation to community agencies.
2. Provides crisis intervention as needed.
A. Responds to emergencies as needed. Assure clients' safety and welfare through adherence to Emergency and Safety Policies and Procedures.
B. Ensures the safety and well-being of clients while transporting to Center and other appointments.
3. Maintains and provides liaison and education for the Center, other community agencies, and the general public.
A. Presentations are presented in an effective and professional manner.
B. Presentations are accurately targeted toward audience.
C. Effectively serves as a positive advocate for Mental Health Services in the area.
4. Maintains and improves professional skills.
A. Attends and actively participates in professional workshops, inservice training, and Quality Assurance review.
B. Reads current professional periodicals.
C. Effectively utilizes people on staff for consultation.
5. Works effectively with support personnel.
A. Provides accurate, timely, and precise information to support personnel.
B. Maintains effective working relationships with support personnel.
6. Completes other related duties as assigned by immediate supervisor.
A. Will execute these duties and responsibilities in a timely and accurate manner.
B. These duties will be treated with the same importance in relation to confidentiality and priority as other duties and responsibilities.
C. Involvement in Consultation and Education activities (at least two per year).
