We are expanding our team to serve the needs of our community in the Lake of the Ozarks, Missouri area!  Central Ozarks Medical Center (COMC) is excited to announce an opening for an OB/GYN & Pediatric Registered Nurse Coordinator position for our Osage Beach Clinic!


COMC is a Missouri non-profit, federally qualified health center that has served Pulaski, Laclede, Camden and Miller counties for over 40 years.   Our goal is to provide the highest quality, affordable healthcare to the communities we serve. Responsibilities:  The Community Resource and Care Coordinator plays a pivotal role in improving the coordination and quality of care for individuals, promoting better health outcomes, and enhancing the overall efficiency of the healthcare system.  

                                                                                                                                   

Essential Duties and Responsibilities:


  • Serves as PrEP/PEP Navigator for all COMC patients and maintains a robust tracking system of all patients newly diagnosed with HIV and New and established patients receiving PrEP.
  • Provide HIV education to members of the community and members of the COMC team as part of infection prevention and reducing stigma.
  • Works comfortably with target populations including homeless, LGBTQIA, and HIV positive groups.
  • Participates and helps plan and organize in activities relating to funded projects (e.g., COVID, HIV, and SUD).
  • Collaborates with other Outreach COMC team members to develop and maintain a comprehensive database of community resources, including social services, healthcare facilities, educational programs, employment services, housing assistance, and other relevant resources.
  • Collaborates with local health departments, school districts, and hospitals to ensure access to resources.
  • Connect patients to relevant internal and external resources, with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care cost.
  • Work proactively, independent of constant supervision, to meet organizational goals.
  • Increase utilization of preventative care for assigned patient population to include Medical, Dental, and Behavioral Health services.
  • Reduce emergency room utilization and hospital readmissions through engagement and education.
  • Increase healthy living comprehension by providing culturally, linguistically, and socioeconomically appropriate education.
  • Improve patients’ ability to self-mange chronic illness and participate in shared decision-making.
  • Manage engagement activities for assigned patients to close gaps in care and increase HEDIS/STAR scores.
  • Other duties as assigned.

                                                                                                                       

Qualifications Requirements:  To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.  The requirements listed below are representative of the knowledge, skill, and/or ability required.  Must be BLS or CPR certified or become certified during probationary period and maintain certification throughout employment with COMC.  Must maintain CEUs as directed by COMC.

 

Education and/or experience: High school diploma or GED required; Must have completed an accredited Medical Assistant Certification program and be a Certified Medical Assistant (CMA) or must earn CMA certification within 90 days of starting the role.  Experience with an electronic health record is preferred. Clinic or hospital experience preferred.

Language Skills:  Ability to respond to inquiries or complaints from customers and regulatory agencies.

 

Mathematical Skills:  Knowledge of fundamental math, adding machine, calculator, and Microsoft office.

 

Reasoning Ability:  Ability to define problems, collect data, establish facts, and draw valid conclusions.

Physical Demands:  The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. This includes moderate physical effort consisting of occasionally lifting or moving over 50 pounds of weight.

 

Travel Requirement: To fulfill the responsibility of the position travel to all COMC locations will be required.  Employee should have valid Missouri driver’s license. Overnight travel may also be required in specific incidences of training, conferences, etc.

Knowledge of, and compliance with, HIPAA and 340B regulations is required. Confidentiality of patient information is mandatory.
