Overview
Working with the Medical Team, delivers medical care for specific patients with Substance Use Disorders (SUD), including formulating a diagnosis and a treatment plan and providing continued follow up patient care. Serves as subject matter expert for the facility’s Medication Assisted Treatment (MAT) program.
Responsibilities:

- Provide direct primary and acute addiction care services to adult patients
- Perform initial and on-going assessment of patients' medical, physical and psychosocial status
- Obtain and evaluate health and medical history; perform a physical exam, conduct preventive screening procedures based on age and history
- Identify medical and health risks
- Complete accurate and timely documentation of observations, interventions, plan of care, referrals and follow-up
- Formulate appropriate differential diagnosis based on history, physical examination and diagnostic testing
- Identify needs of the individual, family or community and works in collaboration with medical and SUD team to develop individualized treatment plans.
- Make appropriate referrals to other health professionals and community agencies
- Reassess and modify the plan as necessary to achieve medical and health goals
- Develop a patient education plan relevant to the condition and patient/ family needs
- Participate in performance improvement activities as assigned
- Mentors and coaches other health team members.
- Orders and interprets diagnostic tests.
- Writes orders for medical medications and treatments, under guidelines of licensure.
- Maintains and follows patient care protocols.
- Works with members of the healthcare team in educating the patients of their medical care.
- Continually updates clinical knowledge and skills through consultation, formal and informal education and review of literature for the benefit of themselves as well as for the healthcare team.
- All other duties as assigned.
Qualifications
- A current CPR/BLS card with demonstrated and cognitive skills is required.
- Must possess a Master’s Degree in Nursing, MD or DO.
- Must have current and valid Nurse Practitioner or Advanced Practice Nurse license with at least one year experience of providing direct patient care.
- Must have prescriptive authority, including DEAX
- A minimum of one year experience treating patients with substance abuse disorders.
- Computer Literacy
- Previous experience in a correctional setting preferred.
