Community Nurse

Job Overview:
We are seeking a dedicated Community Nurse to join our healthcare team. The ideal candidate will provide compassionate in-home care and support to Seniors and other vulnerable persons residing within the Lake Chelan Valley. This job requires a RN degree and Personal Liability Insurance. Hourly rate is negotiable, DOE.

Duties:
- Conduct patient assessments and develop individualized care plans
- Collaborate with healthcare professionals to ensure comprehensive patient care

- Refer Patients to additional community resources as required
- Educate patients and families on health management and disease prevention
- Maintain accurate medical records and documentation

Experience:
- Strong understanding of HIPAA regulations
- Experience in case management and patient assessment
- Knowledge of medical procedures and protocols
- Proficiency in physiology and anatomy
- Familiarity with electronic health record systems
- Previous experience in healthcare setting

Pay: $35.00 - $45.00 per hour
