Permanent Supportive Housing Case Manager
Responsibilities
Provide supportive services to homeless young adults presenting with substance use disorders and mental health concerns.
Coordinate targeted outreach activities about the programs and services available
Conduct screening and assessments to determine program eligibility and identify needs.
Assist clients in locating housing that meets their specialized needs/accommodations, and conduct HQS inspections of housing units to ensure habitability.
Create, monitor, and evaluate individualized service plans.
Provide tenants with an orientation to their apartment and neighborhood including their rights and responsibilities as lease holders during the move-in process.
Provide tenants with opportunities to build skills through facilitated workshops or external linkages.
Coordinate care with community partners and teach young adults how to advocate for services.
Engage prospective landlords in a housing partnership.
Transport youth to important appointments or provide alternative transportation as needed.
Collect and enter required data into the Homeless Management Information System.
Assist clients in calculating and submitting their share of their rent and utility expenses
Engage clients in client satisfaction surveys and focus groups
Attend and actively participate in case disposition and staff meetings to coordinate care.
Other duties as assigned by supervisor.
Qualifications
A minimum of a Bachelor’s Degree in Social Work, Psychology, or another human behavioral science.
A minimum of 2 years of experience providing direct services to at-risk youth and/or vulnerable families
Ability to work effectively with emotionally and behaviorally challenged youth. Clear evidence of trauma-informed care.
Able to demonstrate cultural humility (street youth, pregnant/parenting youth, LGBTQI2 youth)
Working knowledge and effective use of community resources. Comfortable making cold calls to quickly connect to necessary resources in order to resolve housing and other crises.
Hours: 37.5 hours per week, flexible schedule, On call 24/7 one week per month
