GENERAL DESCRIPTION:
The Family Assessment Worker is responsible for initial identification, referral and coordination activities for high-risk families of newborns. This position reports directly to the Family Assessment Supervisor.
SUMMARY OF ESSENTIAL JOB FUNCTIONS:
Provide direct support to prenatal and postpartum families identified as high risk.
Conduct the screening process for initial identification in various hospitals in the community.
Receive daily referrals from hospital personnel as well as other agencies.
Administer the Family Resilience & Opportunity for Growth assessment with all eligible families to determine risk level and need for services.
Obtain permission for Family Support Staff to visit the home.
Ensure referral to the Family Support Specialist for continued follow-up and support services.
Maintain client records in accordance with program and funding source requirements.
Meet weekly with Family Assessment Supervisor to discuss and review each referral.
Attend and participate in all scheduled Healthy Families seminars, conferences, in-service trainings and parent events.
Make DCS referrals as appropriate.
Provide community presentations and community outreach activities.
Participates in agency Continuous Quality Improvement activities/initiatives.
Maintain professional positive relationship with hospital staff and other collaborative agencies.
Perform other related duties as assigned.
REQUIREMENTS:
Minimum of a high school diploma or equivalent required (college coursework preferred), up to a Bachelor’s degree, depending on funders’ requirements.
Two years’ experience providing services to at-risk children and families preferred.
Ability to establish trusting relationships and interact effectively with families.
Knowledge in family life skills.

If not bilingual, able to describe program in Spanish (script will be provided).
Capacity to meet agency/programmatic policies and procedures and meet department performance and productivity standards.
Meet hospital MOA requirements for entry and patient screening.
Certification in CPR/First Aid and/or ability to obtain.
Must be able to provide direct service to the designated client base in the Tucson metro & surrounding area.
Maintain a functional and safe home working space with adequate internet connection for the job.
Comply with all agency and program policies and procedures including confidentiality, employee conduct, computer usage and dress code.
Attend required agency and program/department meetings.
Adhere to the schedule agreed upon with the supervisor.

Eligible for and able to obtain an Arizona Department of Public Safety Level 1 Criminal Clearance fingerprint card.
Must have a valid Arizona driver’s license, vehicle and meet agency liability insurance requirements.

ABILITY REQUIREMENTS:
Sitting for extended periods of time daily; standing and walking short distances.
Moderate twisting, bending, stooping and lifting up to 40 pounds in the performance of assigned duties.
Ability to use a variety of office equipment and machines as referenced.
Normal manual dexterity and eye-hand coordination required.
Repeated hand-wrist movement required.
