A Brief Overview
Perform accurate, timely and complete collection and documentation of patient referral information from external sources. Communicate and coordinate referral information with all home care disciplines and referral sources as required to provide continuum of care from hospital to home. Timely communication with existing home care patients to meet their ongoing needs for enteral feeds, respiratory supplies, and durable medical equipment. Coordinate and document reorders, physician orders, and delivery schedules for assigned patients.
What you will do
- Responsible for taking accurate and complete referrals to the home care department.
- Responsible for completing all data entry of received information into the home care information system.
- Responsible for hand-off communication of all referrals to the involved home care disciplines.
- Responsible to complete initial referral requirements, including but not limited to confirmation of insurance coverage, obtaining physician orders, supply order entry, coordination of delivery timing and place, etc with the interdisciplinary team
- Responsible for timely updating of all patient information in the home care information system
- Responsible for proactively contacting all assigned home care patients to assure that their reorder needs are met timely and accurately.
- Responsible to work with the warehouse operations team to assure that patient’s reorders needs are met timely and accurately
- Responsible for timely and accurate patient information, reorder entry and note documentation into the home care information system
- Responsible to coordinate with biomed and clinical team to ensure preventative maintenance on durable medical equipment occurs before due date
- Responsible for coordinating the patient discharge process for assigned patients per CHHC/CHOP policy and all regulatory and accrediting agency guidelines
- Perform other duties as assigned by Home Care management
Education Qualifications
- High School Diploma / GED Required
Experience Qualifications
- At least two (2) years related experience in medical supply, home health, patient service and/or customer service work Required
Skills and Abilities
- Computer literacy and data entry skills required.
- Strong oral and written communication skills.
- Strong organizational skills.
